Flu Immunisation
Enhanced Healthcare Services worked with a PCT project management team on delivering flu immunisation services across their area to increase their uptake. We set up “Mega” clinics through an active patient management approach. The PCT achieved 78% across their practices, increasing their uptake by 9% that year.
Smoking Cessation
Enhanced Healthcare Services was contracted to provide the central stop smoking service for PCT’s to sign post individuals who want to stop smoking to the local providers like pharmacists, GP’s, Group Sessions and One to Ones. This service operates 7 days per week. We offer an incoming and outgoing call service.
Childhood Immunisation
Enhanced Healthcare Services audited childhood immunisation records extracting data from approximately 300 practices across London based inner city PCT’s, then reconciling this data against immunisation data held centrally to identify unimmunised children so the PCT to start a “catch up” campaign
Men’s Health/Male Life Expectancy/CVD Risk Assessment
Enhanced Healthcare Services was commissioned to extract data from 200+ GP practice systems and provide performance management data on disease registered patients and non disease registered patients around the following data fields:
Enhanced Healthcare Services was subsequently commissioned an active management contract to arrange men’s screening appointments across specialist clinics operating evenings and weekends.
Service Planning
Enhanced Healthcare Services supported a Provider arm of a PCT in writing their Business Case and Care Pathways to their commissioning department for the award of a community based Diabetes contract. This contract was a joint bid with the local acute trust to reduce all new and follow up Diabetes caseloads going through secondary care organisations.
Maternity Service Mapping Enhanced Healthcare Services was commissioned to map the local Community Maternity service provision across various PCT’s to identify potential efficiencies and improvement in productivity.
